Form and Survey Creation
Templates > Forms/Surveys > Create

Templates are a great place to start when thinking of sending out a campaign. 

When you create a campaign and select a form, survey, event template or email template, it’s good to understand they are copies of the template you originally created. The original template located under Templates > Forms/Surveys will remain untouched and vice versa.
Note: The process for creating a form, survey and event template is identical. The reason for the separation is purely due to aid the selection process during campaign creation.
For this example, we will create a form.

Templates > Forms > Create Template > Give your form an internal Name, Category and Sub Category > Create:

The Category and Sub Category fields are internal only and can be a way to help with the management and filtering of forms/surveys in the platform. They can be updated at any time. 

Your blank form is now ready for population with the field options available to be dragged and dropped onto the main form  field area:

Text and Image Area
At the top of the form there is an area where text and images can be added. Text can be typed in and formatted using the black edit bar, and images can be inserted by clicking the 'Insert Image' button - third from right.

Drag and drop fields onto the form template. You can have 3 columns width ways and also stacked, one on top of each other. Re order the fields by clicking into the field and dragging and dropping it into place by the arrow icon.
Note: The text and image area is not available on the event form.
Part Responded
Create forms and surveys over multiple pages. Under Standard Fields use the New Page Option to create a new page at the bottom of the existing page:

The New Page field can then be populated with CRM/Standard/Attribute fields as normal.  In this example the contact details have been put on one page and other questions on the next:

Creating a form or survey over multiple pages may help survey fatigue.  If the contact drops out the form or survey after clicking Next on the first page, the data will still be collected and stored as 'Part Responded'.  Each time the contact moves between the pages onto the next one, clicking the Next button, the previously entered data will be saved.

This is versus creating the form or survey on one page and the contact dropping out before they finish clicking the Next button so no data is saved. 
Save for Later
Enable the ability for contacts to save a form submission for completion at a later date. Clicking this option will send the submitter the link to the form to their email address. 
Note: When dragging and dropping a field option, ensure the field space turns blue before you drop the field onto it.

CRM Fields

Using CRM fields in a form or survey will create the contact automatically in the platform, upon submission. All future submissions using the CRM Fields will update the contact accordingly.

Note: You must use a CRM email address alongside other fields to create a new contact in the platform

Pre-populate and cleanse your arkflux contact data

This feature brings two new use cases to the platform, applicable to CRM fields only:

1. Cleanse your arkflux contact data by sending a periodic email with a form that displays their information ready for editing.

2. Simplify form submissions by automatically populating the form with the data stored on their contact record.

To switch this feature on, simply drag over your CRM fields and select the Pre-populate Field option on the left.

Make sure you set the fields to mandatory as required.

Attribute Fields

Attribute Fields are questions of importance to the business, created in the platform to include in forms and surveys. Upon answering, these fields will show against the contact profile with all future submissions using the attribute fields will update the contact accordingly.

These fields are also used when creating hyper targeted marketing lists with dynamic segmentation .

Standard Fields

If the field you require doesn’t exist in CRM Fields, you can select an appropriate field in Standard Fields and edit as required.

There are 36 Standard Fields to choose from and two main types; fields that are ready for use immediately upon selection e.g. Text Field, Date and Time, and the fields that need options entering upon selection e.g. Checkbox, Radio Button, Number Slider.

Adding Values to a Field
On fields that have multiple choice answers, manage these field options by clicking on the field to access the left hand blade. Click [Add] on the left hand blade and enter value one per line:

Field Title and Help Text
Edit these areas for each field by clicking on the field to access the left hand blade

Placeholder Text
Text contained within the field itself that acts as a hint to what to enter and will disappear once the contact starts to fill in the field.
API Reference
The API Reference is used for third party apps that will receive the form via a web hook.
Hidden Field
Fields that are used to pass values stored on the form.
Contact Person
You can identify a CRM field on a form by hovering over the number in the field. The numbers themselves are there to identify individual sets of contacts led by CRM email address fields e.g. contact details 1, contact details 2. This allows the data of multiple contacts to be collected on one form and created in the platform. 
Mandatory
Select if a field has to be completed before the form can be submitted. 
Conditional Logic
The idea that you can set rules, or conditions, that cause your process to change based on input. So in relation to form and survey fields, rules can be applied to fields to personalise the contacts experience.

An example of a common conditional logic rule is "show field entitled "Other" if "Other is selected in a previous question.

Conditional Logic will be visible in the left hand blade when you click on the field:

Styling

The styling tab is used to make visual changes to the form:

Theme
Change the theme of the form to reflect your brand. This can include submit buttons and fields.

Font Family and Size
Change the font to reflect your brand. 

Text Colour
The colour of the main text content.

Link Colour
The colour the hyperlinks within the form will change to once linked.

Answer Colour
The colour of the answers the contact will type into the fields.

Foreground
The colour of the foreground of the form.

Background 
The colour of the background of the form.

Button Alignment
Choose  the position of the submit button.

Button Hover Off
The colour of the submit button when Off.

Button Hover On
The colour of the submit button when On.

Submit/Next Button Text
The text on the submit button.

Back Button Text
The text on the button that navigates Back through form pages.

Form Drop Shadow
A single line shadow around the outside of the form.

Create forms and surveys and include paid products and services for contacts to select and submit payment via Stripe.
Product Selector
Use the Product Selector field located under the CRM Field tab to add the products and services to the form:

The products and services offered are created via the Line Items created under Sales > Line Items.
